CBHC Program Information

What is the purpose of the CBHC program?
The purpose of the CBHC is to meet the needs of individuals with timely, flexible,
treatment for mental health, substance use, and co-occurring disorders including
routine appointments, urgent visits, and 24/7 community-based crisis intervention as
an alternative to hospital emergency departments.
We are open daily for walk-ins, urgent & routine appointments, and
provide around-the-clock care for anyone in need of crisis support. We have in-person &
telehealth options for all services.

How does the CBHC program help?
The CBHC helps by providing:
- Integrated mental health and addiction treatment
- Extended hours, including evenings and weekends
- Same-day access to intake and brief assessment, urgent and crisis treatment including medications, and drop-in treatment and support (e.g., group sessions, peer supports)
- 24/7 mobile and community crisis response with Community Crisis Stabilization for youth and adults
- Telehealth and flexible service delivery locations (e.g., home, school, etc.)
- Peer supports
- Care coordination
- Ability to serve all ages, including child and family-specific treatment models and models for older adults
Our services include urgent & routine outpatient appointments, crisis intervention services, medication management, peer & recovery support, and care coordination.

Who makes up the CBHC program?
A team of Care Navigators, Clinicians, including Crisis Intervention Workers, Recovery and
Peer Supports, Nurse and Psychiatry, Mental Health Workers, Family Partners, Case Managers,
and Clinician Assistants.
Our team members are compassionate, sensitive, & trauma informed. You may meet with a
Care Navigator, Clinician, or a Clinician Assistant. Our team also includes Recovery &
Peer Supports, Nurses, and Psychiatrists.
